Redeem Integrative Health is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to follow the terms of the notice currently in effect.

• Treatment. We share information with laboratories, compounding pharmacies and, at your direction, other clinicians involved in your care.
• Payment. As a direct-pay practice we process your payments directly. We do not submit claims to insurance carriers or provide superbills for clinical services.
• Healthcare operations. We use PHI for quality review, clinical documentation, scheduling, training and administration of the practice.

We may use or disclose PHI without your authorization where the law permits or requires it — including public health reporting, communicable disease control, suspected abuse or neglect, health oversight activities, judicial or administrative proceedings, law enforcement requests, serious threats to health or safety, workers' compensation, and specialized government functions.

Most uses of psychotherapy notes, any marketing use of your information, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

• Access and copies. Inspect and obtain a copy of your records, typically in an electronic format through the patient portal.
• Amendment. Request correction of information you believe is inaccurate or incomplete.
• Accounting of disclosures. Request a list of certain disclosures we have made.
• Restrictions. Request limits on how we use or share information; we will accommodate reasonable requests where feasible.
• Confidential communications. Ask that we contact you at a specific phone number, email or address.
• Paper copy. Receive a paper copy of this notice, even if you agreed to receive it electronically.

Clinical communication takes place through our HIPAA-compliant portal. Standard email and text messages are not fully secure; if you choose to communicate that way for convenience, you accept the associated risk, and we will keep such messages limited to scheduling and general logistics.

If you believe your privacy rights have been violated, you may file a complaint with our practice through the contact page, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will never be penalized or retaliated against for filing a complaint.

We may change this notice and make the new terms effective for all PHI we maintain. The current version is always posted on this page with its effective date.
