Telehealth Consent

Informed care,delivered remotely

Most care at Redeem Integrative Health is delivered through secure video consultations. This page explains how telehealth works, what it can and cannot do, and the consent you provide before your first visit.

01What telehealth means here

Telehealth is the delivery of clinical care using secure, HIPAA-compliant video and messaging rather than an in-person office visit. Your consultation with Jordan Montoya, MSN, FNP-C, follows the same clinical standards as an in-person appointment: history, symptom review, laboratory interpretation, diagnosis where appropriate, and a documented treatment plan.

02Benefits

  • Access to specialized integrative care from anywhere in Texas, without travel.
  • Unhurried 45-minute initial consultations scheduled around your life.
  • Direct, secure messaging with your clinician between visits for members.
  • Faster coordination of laboratory orders, prescriptions and supplement plans.

03Limitations and risks

  • A remote visit cannot include hands-on physical examination; some conditions require an in-person evaluation, and we will tell you when that is the case.
  • Technology can fail. Poor connectivity or device issues may delay or interrupt a visit, and the appointment may need to continue by phone or be rescheduled.
  • Rarely, information transmitted electronically may be insufficient for sound clinical decision-making, requiring additional testing or referral.
  • As with any electronic system, a security breach is possible despite encryption and safeguards.

04Your responsibilities during a visit

Please join from a private, well-lit location where you can speak freely, using a device with a working camera and microphone and a stable connection. Be physically located in Texas at the time of your visit, have a list of current medications and supplements available, and confirm the address and pharmacy details we hold for you.

05Privacy and recording

Visits take place on a HIPAA-compliant platform. Documentation from your visit becomes part of your medical record and is protected under the same laws as in-person care. We do not video-record consultations, and patients may not record a visit without the clinician's written permission.

06Emergencies and after-hours care

Telehealth is not emergency care. For urgent or life-threatening symptoms, call 911 or go to the nearest emergency department. Outside clinical hours, contact your primary care provider, an urgent care clinic or emergency services rather than the portal.

07Your rights

Consent to telehealth is voluntary. You may withhold or withdraw consent at any time without affecting your right to future care, request an in-person appointment in Amarillo where clinically appropriate, ask questions about your care at any point, and expect the same confidentiality you would receive in an office setting.

08Consent

Before your first visit you will be asked to acknowledge, within the patient portal, that you have read and understood this information, that you consent to receive care by telehealth, and that you understand the benefits, risks and alternatives described above.
