• Memberships renew on the stated monthly or annual cycle until cancelled through the portal or in writing.
• We ask for advance notice of cancellations or reschedules so that appointment time can be offered to another patient; the notice window and any associated fee are shown at booking.

You agree to provide accurate, complete health information, to disclose all medications and supplements you take, to complete laboratory testing as clinically directed, and to maintain a relationship with a local primary care provider for routine preventive care and emergencies.

Our scheduling, records, dispensary and laboratory functions are delivered by third parties including Healthie, Fullscript, Quest Diagnostics and Labcorp.
